Middle School Winter Camp 2018!
Thank you for registering for Winter Camp 2018!
Please be sure to answer all of the questions below.
Note: If your student will be taking medications during Winter Camp 2018, you must give all medications to your student's small group leader when you check-in your student. Bring your student's medication in a ziploc bag with their name and instructions on it.
Every possible safety precaution will be taken by those in charge and every possible attempt will be made to contact the parent or guardian immediately in the event of injury or other emergency.

I hereby give consent for my student to attend Harvest’s Middle School Winter Camp 2018 held February 9-11, 2018. In the event that he or she is injured while under the care of the Harvest Bible Chapel and its’ representatives and requires the attention of a doctor, I hereby consent to and will be responsible for any reasonable medical treatment as deemed necessary by a licensed physician.
I further agree to hold the licensed physician, the medical facility, the Harvest Bible Chapel and its’ representatives free and harmless of any claims, demands, or suits for damages arising from the authorization and provision of such medical treatment.
I understand the nature of the events and do hereby release the Harvest Bible Chapel and its’ representatives from any liability due to accident or injury incurred by my child.
All students and family members attending Harvest student events hosted by Harvest Bible Chapel give consent that any images or likenesses of students or family members may be used for promotional purposes or in promotional materials for Harvest Bible Chapel or its ministries.
